We collect and use the following information to deal with an enquiry, provide care, a comprehensive nutritional service and to ensure accurate record keeping with consent. Information we collect includes but not limited to:
- Name, address, contact details, gender, emergency contact, details of GP and other medical professionals, gender, occupation.
- Current and past medical history, current medications, body measurements, and lifestyle information.
- By accessing the website and relevant sites information such as internet protocol address, your log in information, browser type, browser plug in types, diagnostic data, operating systems, time zone.

3. Information gathering.
The information required to carry out any service by NC Nourish will come directly from you with your consent.
4. Data retention.
Your details will be held for 8 years from the year of last entry; this is in line with The Records Management Code of Practice for Health and Social Care (2021/2023) and the British Dietetic Association guidance for record keeping. The records and information will be collected, assessed and documented by Naomi Cockram.
5. Sharing personal data.
You agree that we have the right to share your personal data with;
- Relevant health care professionals if through discussion and with your consent it is agreed it is in your best interest.
We are subject to a common law duty of confidentiality. However, there are circumstances where we will share relevant health and care information. These are where:

- on a case-by-case basis, the public interest to collect, share and use the data overrides the public interest served by protecting the duty of confidentiality (for example sharing information with the police to support the detection or prevention of serious crime);

NC Nourish are committed to protecting your data. IONOS, (Worcestershire, England) is the host website however no data is stored on this sight. Laptops and documents with your information are password protected.
NC Nourish may include links to third party websites, we do not control these sites and therefore are not liable for their privacy policies or terms and conditions. Please review these prior to using any link.
